RELEASE AND WAIVER OF LIABILITY AGREEMENT I, (“Participant”), acknowledge that I have voluntarily applied to participate in the following activities at the November 17, 2024 Valley View Elementary School Booster Club hosted Fall Fest, held at 1201 South Capital of Texas Highway, Austin, Texas 78746. By signing this release, I acknowledge that I am voluntarily participating in this activity and assume any and all risks of bodily injury or property damage, whether those risks are known or unknown. As consideration for being allowed to participate in the Valley View Elementary School Fall Fest on November 17, 2024, I hereby forever release the Valley View Elementary Booster Club and their respective directors, officers, employees, volunteers, agents, contractors, and representatives (collectively “Releasees”) from any and all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse, and legal representatives now have, or may have in the future, for injury, death, or property damage, related to: My participation in these activities, The negligence or other acts, whether directly connected to these activities or not, and however caused, by any Releasee, or The condition of the premises where these activities occur, whether or not I am participating in the activities at the time. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse, and legal representatives will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by this release. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE VALLEY VIEW ELEMENTARY SCHOOL BOOSTER CLUB, AND I SIGN IT OF MY OWN FREE WILL. If Signed by Parent or Guardian: I verify that the dangers of the activities and the significance of this Release and Waiver were explained to the Participant, and the Participant understood them. ELECTRONIC SIGNATURE ACKNOWLEDGEMENT AND CONSENT FORM I, [Name], agree and understand that by signing the Electronic Signature Acknowledgment and Consent Form, all electronic signatures are the legal equivalent of my manual/handwritten signature, and I consent to be legally bound to this agreement.
